Childhood intellectual disability and parents' mental health: integrating social, psychological and genetic influences

Background
Intellectual disability has a complex effect on the well-being of affected individuals and their families. Previous research has identified multiple risk and protective factors for parental mental health, including socioeconomic circumstances and child behaviour.
AimsThis study explored whether genetic cause of childhood intellectual disability contributes to parental well-being.
MethodChildren from across the UK with intellectual disability due to diverse genetic causes were recruited to the IMAGINE-ID study. Primary carers completed the Development and Well-being Assessment, including a measure of parental distress (Everyday Feeling Questionnaire). Genetic diagnoses were broadly categorised into aneuploidy, chromosomal rearrangements, copy number variants (CNVs) and single nucleotide variants.
ResultsCompared with the UK general population, IMAGINE-ID parents (n = 888) reported significantly elevated emotional distress (Cohen's d = 0.546). Within-sample variation was related to recent life events and the perceived impact of children's difficulties. Impact was predicted by child age, physical disability, autistic characteristics and other behavioural difficulties. Genetic diagnosis also predicted impact, indirectly influencing parental well-being. Specifically, CNVs were associated with higher impact, not explained by CNV inheritance, neighbourhood deprivation or family structure.
ConclusionsThe mental health of parents caring for a child with intellectual disability is influenced by child and family factors, converging on parental appraisal of impact. We found that genetic aetiologies, broadly categorised, also influence impact and thereby family risks. Recognition of these risk factors could improve access to support for parents, reduce their long-term mental health needs and improve well-being of individuals with intellectual disability.

Summary
This chapter examines how self-determination theory’s concept of basic psychological needs, and the interpersonal processes and mechanisms that stem from mutual need fulfillment, offer a novel and integrative perspective on optimal relationship development. We first present self-determination theory’s perspective on basic psychological needs. We then discuss how need fulfillment facilitates true-self involvement and self-determined relationship motivation, along with their downstream relationship benefits. Finally, we offer an integration of SDT with several major theoretical perspectives, concepts, and mechanisms on optimal close relationships (presented in this volume) to illustrate the utility and explanatory power of this framework.

Background
Long-acting injectable formulations of antipsychotics are treatment alternatives to oral agents.
AimsTo assess the efficacy of aripiprazole once-monthly compared with oral aripiprazole for maintenance treatment of schizophrenia.
MethodA 38-week, double-blind, active-controlled, non-inferiority study; randomisation (2:2:1) to aripiprazole once-monthly 400 mg, oral aripiprazole (10–30 mg/day) or aripiprazole once-monthly 50mg (a dose below the therapeutic threshold for assay sensitivity). (Trial registration: clinicaltrials.gov, NCT00706654.)
ResultsA total of 1118 patients were screened, and 662 responders to oral aripiprazole were randomised. Kaplan–Meier estimated impending relapse rates at week 26 were 7.12% for aripiprazole once-monthly 400mg and 7.76% for oral aripiprazole. This difference (−0.64%, 95% CI −5.26 to 3.99) excluded the predefined non-inferiority margin of 11.5%. Treatments were superior to aripiprazole once-monthly 50mg (21.80%, P⩽0.001).
ConclusionsAripiprazole once-monthly 400mg was non-inferior to oral aripiprazole, and the reduction in Kaplan–Meier estimated impending relapse rate at week 26 was statistically significant v. aripiprazole once-monthly 50 mg.

Summary
Executive Summary
Renewable energy (RE) has the potential to play an important and increasing role in achieving ambitious climate mitigation targets. Many RE technologies are increasingly becoming market competitive, although some innovative RE technologies are not yet mature, economic alternatives to non-RE technologies. However, assessing the future role of RE requires not only consideration of the cost and performance of RE technologies, but also an integrative perspective that takes into account the interactions between various forces and the overall systems behaviours.
An increasing number of integrated scenario analyses are available in the published literature. They are able to provide relevant insights into the potential contribution of RE to future energy supplies and climate change mitigation. A review of 164 scenarios from 16 different large-scale integrated models was conducted through an open call. Although a collection of scenarios from the literature does not represent a truly random sample suitable for rigorous statistical analysis, a scenario overview can provide some critical and strategic insights about the role of RE in climate mitigation, in spite of the uncertainties involved.
Although it is not possible to precisely link long-term climate goals and global RE deployment levels, RE deployment significantly increases in the scenarios with ambitious greenhouse gas (GHG) concentration stabilization levels. Ambitious GHG concentration stabilization levels lead on average to higher RE deployment compared to the baseline. However, for any given long-term GHG concentration goal, the scenarios exhibit a wide range of RE deployment levels.

Expectations for service provision following a disaster are understandably high but difficult to meet. This report considers the case of a patient with pre-morbid anxiety traits and asthma who suffered organic brain damage as a consequence of the Hillsborough Football Stadium disaster, whose care was reactive rather than proactive and resulted in his compulsory admission to a psychiatric hospital. Aftercare arrangements need to be anticipated in advance and should involve early psychiatric assessment of at-risk patients. Medical control of case management following disasters needs further clarification.
